Erie County Community Health Center Recognized as Patient-Centered Medical Home
Sandusky, OH – October 21, 2024 – The Erie County Community Health Center (ECCHC), a Federally Qualified Health Center (FQHC) and part of the Erie County Health Department, is proud to announce that we have again received Patient-Centered Medical Home (PCMH) recognition for Primary Care from the National Committee for Quality Assurance (NCQA).
This recognition, coupled with ECHD/ECCHC’s recent HRSA Gold Award for Quality Leadership, signifies to our community that we provide the comprehensive care they need.
The NCQA is the leading accrediting body that sets standards for healthcare organizations, including hospitals, health plans, and medical practices. PCMH accreditation is a national recognition that underlines ECCHC’s commitment to providing high-quality, patient-centered care. This accreditation demonstrates that ECCHC meets rigorous standards for patient-centeredness, access to care, quality of care, and coordinated care.
