In our newest episode of 2-Minute Health Break, we’re honored to share a meaningful patient success story featuring Savannah, a new mom, her newborn, and the care team that supported her throughout her prenatal journey.
Savannah began her experience with a pregnancy test and continued receiving prenatal care with Hannah Schoen, FNP-BC, at the Erie County Community Health Center. Along the way, she was connected to additional resources and support services that helped her feel prepared, confident, and cared for every step of the way. This episode highlights what it means to provide care that is truly closer to care — bringing together medical services, community support, and a team committed to helping families thrive.
