Do I need to make an appointment?

Yes, you will need to call 567-867-5174 to schedule an appointment and find out what documentation you will need to bring to your appointment.

Do you accept private insurance?

Yes. We accept Medicaid and most Medicaid HMOs, including Buckeye, United Healthcare, Care Source, Molina and Paramount. Please call your insurance carrier to check that we are a provider for your insurance. 

Is this Health Center free?

We are not a free clinic. We offer a sliding fee discount program (based on income) for patients without insurance or who have limited insurance. We can assist with Medicaid applications for those who appear to be eligible. Payment plans are available.

How does the sliding fee scale work?

To qualify for the sliding fee discount program, you will be required to fill out an application and provide proof of income for all family members living in your household. Our scheduler will tell you what paperwork you will need to provide to qualify for a reduced fee for services. These may include:

• one month of the most recent pay stubs
• bank deposits
• social security determination letter
• prior year tax return
• W-2 
• completed sustainability form
