The Erie County Community Health Center provides the following value services:

Certified Application Counselors (CAC):  Contact one of the CACs for assistance applying for Medicaid, Health Insurance through the Healthcare Marketplace or for questions regarding Medicare.  Call 419-626-5623, Ext. 120 to speak with one of our Certified Application Counselors.

Community Health Worker (CHW):  The CHW assists in meeting the needs of our community members and patients.  This member of the health care team works to achieve the following:

  • Address any barriers to care
  • Provide referrals for such needs as food, housing, education, and mental health services
  • Educate people about the importance and availability of healthcare services
  • Discuss health concerns with community members they serve and facilitate access to the healthcare services
  • Provide informal counseling and social support
  • Advocate for individual and community needs
  • Assist residents with health system navigation
  • Improve care coordination.

Cribs for Kids:  The Erie County Community Health Center is a Cribs for Kids partner and promoter of Safe Sleep for children.  The Health Center’s Community Health Worker provides a Pack N Play, Safe Sleep Assessment and education for mothers-to-be at 32 weeks of pregnancy or beyond and families of an infant less than 1 year of age.  Call 419-626-5623, Ext. 113 to speak with our Community Health Worker if you are in need of a safe sleeping place for your baby.

TB testing:  The Erie County Community Health Center utilizes the Tuberculin skin test for the screening of Tuberculosis exposure for pre-employment, screening or medical reasons.  Call Central Scheduling at 419-626-5623, Ext. 174 to schedule an appointment for a TB test.    

Blood lead testing:   Lead can damage nearly every system in the human body and has harmful effects on both adults and children. The Erie County Community Health Center provides blood lead testing for children less than 6 years of age who are at risk for elevated blood lead levels. Call Central Scheduling at 419-626-5623, Ext. 174 to schedule and appointment for blood lead testing/screening for your child.
