The Erie County Community Health Center is a Federally Qualified Health Center providing comprehensive services in a patient-centered medical home environment. Following the Circle of Care model, the Erie County Community Health Center improves health care in Erie County by transforming how primary care is organized and delivered. It is not simply a place but a model of the organization of primary care that delivers the core functions of primary health care.
Through this comprehensive continuum of care, the Erie County Community Health Center is able to meet the large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care for individuals of all ages. Providing comprehensive care requires a team of care providers. This team includes physicians, advanced practice nurses, dentists, nutritionists, nurses, chiropractors, counselors, social workers, educators, and care coordinators. Additionally, the Erie County Community Health Center has a vast network of specialists, local agencies, and community resources, linking patients to providers and services within the community.
Relationship-based care stands at the center of the Erie County Community Health Center primary care practice, with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences. The Health Center staff actively support patients in learning to manage and organize their own care.
The Erie County Community Health Center coordinates care across all elements of the broader health care system, including specialty care, long-term care, hospitals, home health care, and community services and supports. Health Center staff value clear and open communication among patients and families, the medical providers, and members of the broader care team.
Delivering accessible services to all members of the community means the Erie County Community Health Center offers extended office hours, public health outreach clinics at 11 locations throughout Erie County, and three new community-based Health Center locations at Erie County Jobs & Family Services, EHOVE, and Vermilion High School will be opening in 2019.
